HIPPA

The Health Insurance Portability and Accountability Act, also known as HIPPA, was created by the US Congress. The form gives permission to a healthcare related entity to disclose protected health care information to a third party, such as an attorney representing a plaintiff in a car accident case or a medical malpractice case.
